Make an Appointment

Contact us to make an appointment with our expert reproductive endocrinologists Dr. John Schnorr, Dr. Heather Cook,  Dr. Jessica McLaughlin, Dr. Carrie Riestenberg and Dr. Andrea Peterson. Our doctors offer the latest in high-tech and low-tech reproductive treatment options to help you get pregnant. There are two ways you can make an appointment—schedule online or by phone.

Schedule Online

Perform all the scheduling online by going to www.cfsenroll.com and complete the basic contact information. You will need your insurance information to complete the form. Once complete, you will be given a phone number you can text message so we can find a time that works for you and get you scheduled. Our new patient coordinators are available Monday thru Friday, 9am to 4pm.

Schedule by Phone

Simply call us during business hours and we will get you scheduled. If we are on another call, leave us your name and phone number and we will call you back. The phone number is 843-883-5800.

Second Opinion Consult

Have you seen a Reproductive Endocrinologist in the past? Are you unsure about their recommendations for your evaluation or treatment? Do you feel like you are not getting the level of care you deserve? If so, you owe it to yourself to get a qualified second opinion and Coastal Fertility Specialists is here for you with our national award winning level of care!

Dr. Schnorr, Dr. Cook, Dr. McLaughlin, Dr. Riestenberg and Dr. Peterson are proud to offer a free second opinion consult for any patient who has seen a reproductive endocrinologist in the past and would like a second opinion. Our physicians will spend time with you reviewing your history, testing and treatment to help you better understand the causes of your infertility and your treatment options. Your physician consult will then be followed by a nursing consult and a financial consult. Your consult can be in any of our five offices. There are absolutely no obligations or strings attached.

Complete records are important for a successful appointment and are required for review for a free second opinion. We do not want to duplicate testing unless they are outdated or feel they are inaccurate.

Download the medical records release form and send it to your physician to request a copy of your medical records. It typically takes 2 weeks for a physician’s office to send the medical records.

Please bring any medical records you have with you to the consult.

New Patient Forms

Online Forms

Completing your new patient paperwork online is the simplest way to get the process started. The online paperwork will take approximately 20 minutes. You will need your insurance information and if relevant, your partner’s medical information. Our online portal uses an encrypted connection and is HIPAA secure.

To submit your information online click the save and return link that enables you to create an account that you can return to before your form is complete and submitted. This will save you time if you lose your internet connection or don’t have all of the required information. If you want to bypass the save and return portion and do it all at once click here.

Printing and Emailing/Faxing the Forms

If you are uncomfortable providing your information over the internet or don’t have a reliable internet connection, you can submit your information via our printed forms. You can start your new patient visit by downloading the new patient packet and emailing/faxing it back to us. Once it’s complete, fax or email it to your New Patient Coordinator who can be reached at 843-883-5800.
